Complete guide

What you will get

A report draft that separates delivery, outputs, observed outcomes, tentative interpretation, data limits and follow-up actions without overstating impact.

Before you begin

Prepare the original plan and targets, delivery records, participation or output data, outcome evidence and follow-up issues. De-identify participant records and confirm that comparison units and periods match.

How to use it

  1. Separate targets, actual activities, outputs and outcome evidence.
  2. Check response counts, missing values, units and reporting periods.
  3. Copy the instruction below and complete only known inputs.
  4. Compare every claim with its source and label insufficient evidence.
  5. Confirm actions, owners, dates and confidentiality before circulation.

Copyable instruction

Build an activity and outcome report using only the plan, records and data I supply. If targets or delivery evidence are missing, request them and stop. Do not repeat participant-identifying information.

Plan/targets: {{PLAN}}
Actual delivery: {{DELIVERY}}
Participation and output data: {{DATA}}
Outcome evidence: {{OUTCOME_EVIDENCE}}
Issues and follow-up: {{ISSUES_FOLLOW_UP}}

Separate delivery facts, outputs, observed outcomes, tentative interpretation, data limits, issues, lessons and follow-up actions. Compare target with actual only when units and periods match. Do not invent causes, testimonials, counts, rates, savings or impact. Mark unsupported claims [INSUFFICIENT EVIDENCE]. Finish with an action table, owners, dates and confirmation gaps.

Worked example

Synthetic example: 38 of 40 targeted participants attended and 30 feedback forms were received. The report may state actual attendance and response rate, but it must not report 95% satisfaction without a valid response count or claim long-term change was caused by the programme.

Quality check

  • Targets and actuals use matching units and periods.
  • Response counts and denominators accompany every rate.
  • Outputs, outcomes and impact are not conflated.
  • Tentative interpretation and evidence gaps are clearly labelled.

Limitations and responsible use

Correlation or post-programme feedback does not establish causation or long-term impact. An officer must verify sources, privacy, formulas and every outcome claim before use.
